for Jane DoeIntroduction
As a person who is experienced in living with a disability, as well as being a long-time waivered-service consumer, Jane has a clear understanding of her personal needs, and the supports and services that will diminish those needs. With Jane's specific needs in mind, this plan has been outlined, on her behalf, following the Community Support Plan (CSP). The plan was designed using the guidelines detailed in the Consumer Directed Community Support Consumer Handbook, DHS-4317 5-05. In this plan, Jane is referred to as "Jane."

The following information including name, address, birth date, case mix, waiver type, time period for the plan, budget amounts, case manager information was sent to Jane by her case manager.

Jane is a well-educated, independent-minded individual who wants to live as independently as possible. As a waivered-service consumer, Jane has modified her home to make it more wheelchair-friendly. She has also been able to stay in her home, in part, because of the help of personal and homemaker assistance make possible by waivered-service funds.
During the years that Jane has been a waivered-service consumer, she has helped maintain her family's home, parented the family's children, and complete a Bachelor's degree. She has also ….
